I wanted to do a piece to capture the struggle that people living with mental illness have to deal with every day. The original composition showed the face of the "madness" figure, but since the affliction lives within the person and comes from within, I opted to pose it as a living parasite, attached to its victim, but it's face obscured by the victim's own face.
